Anyone know what’s causing this issue? Only on one side of print. I have rotated object and still get sane issues.
That’s not enough information. You still get the same issue at the same location on the part or you get the same issue on the same area of the build plate? Also no mention of what machine you are using and what slicer software.
Wild guess is something is causing a restriction in the filament movement through the extruder and the hotend is getting starved of plastic. Or, build plate is not level or oils on the build plate.
Speed vs. temperature? I agree that it looks like not enough plastic is coming out at that point.

I’m using a Bambu Lab P1S. I’ve tried printing in different positions on the build plate and with different rotations, but the issue always appears in the same area of the model.
What’s odd is that the opposite side has identical geometry but prints fine. I’ve also tried both with and without a fillet on the edge, and the problem remains. Using standard 0.08 layer height settings.
ok so it does not sound like it’s a buid plate or bed level issue. It could be a model issue, ie not really flat. But before even looking at that, I would not call a layer height of 0.08 as “standard”.
It even looks like you don’t have too many bottom layers specified. I would set the layer height to .2mm and see how that does. And if you really need 0.08mm layers then just set the first layer to a “standard” .2mm and see what that gets you.
I’d consider trying increasing temperature a little bit, maybe 5°C
