It depends. Some PLA likes 190c, some likes 230c…if the fan is on the hot end: yes…all the time.
Depends on your termistor reading and also the pla, check the supplyer homepage for information on your plastic the range for pla is around 190-260degress celsius. The fan helps on bridges and overhangas got it? Use it 
We, in the Mendel90 community, print PLA at 185C on a heated 2mm thick glass plate at 70C. The fan is turned on after the first layer.
I really suggest you test your hotend temperature using an external temp prob (contact, not IR gun). Make sure that when you set it to 190c you actually get 190c. I’ve seen hotends that are ~20c off.
